SECTION 2 - Proof of Identity

To help establish your identity, your application must be accompanied by copies of AT LEAST TWO different official documents which, between them, provide sufficient information to prove your FULL NAME, DATE OF BIRTH, CURRENT ADDRESS and SIGNATURE. For example, a combination of driving licence, medical card, birth / adoption certificate, passport, utilities bill and any other official documents which show the required details. Please scan and upload those documents below. For enquiries relating to Social Services records, photo ID will also be required. For CCTV footage of your vehicle please provide a copy of your V5C log book
